Form I-360, signed by the Authorized Official of the Religious Organization.

Form G-28, signed by the Authorized Official of the Religious Organization.

Abstract of Oral Job Offer from Religious Organization.

A letter from an authorized official of the religious organization in the United States which establishes that the alien has authorization to conduct religious worship and to perform other duties usually performed by authorized members of the clergy including a detailed description of the authorized duties.

Evidence of the religious organizations
federal (not state) tax exempt status, under section 501(a) of the Internal Revenue Code, or a completed application for federal tax exempt status, (Form 1023, Application to U.S. I.R.S. for Recognition of Exemption), with supporting documentation.

Articles of Incorporation/Certificates of Amendment to Religious Organization.

Address List of Officer/Elders/Members of Board of Directors of Religious Organization.

List of Congregation Membership.

Newsletters & Brochures of Religious Organization.

Bank Statements of Religious Organization.

Photos of Members/Staff/Services of Religious Organization.

Local Newspaper Articles regarding Religious Organization.

Translated copies of the Alien's Graduate and Undergraduate Degrees.

Translated Birth Certificate/Family Registry of Alien.

The Alien's Unexpired Passport.

The Form  I-94 (Arrival/Departure Record) of the Alien verifying current valid nonimmigrant status in the U.S.

Copies of Documents/photographs establishing that the alien has been a member of the Religious Organization for the preceding two years.

A copy of the alien's certificate of ordination or authorization as a minister of the religious denomination.

Translated copies of the alien's Certificate of Ordination/Certificate of Belonging, (Translated School Transcripts or copies of Academic Awards are useful, but not required.)
